Please ensure that you have photo ID when picking up letters / prescriptions. You maybe asked to show ID before being given any information. This is for your own data protection.
Please ensure that the surgery has your written consent for anyone other than yourself to discuss anything regarding your medical data. No information will be given without this.
Children will be removed from parental access from the age of 13. Parental access will be reinstated should a clinician feel it is in the interest of the child.
