Please then scan the registration form and send together with one form of photo ID and a recent utility bill showing your current address to Meccg.nchc@nhs.net. Alternatively, please post your completed form and a photocopy of your photo ID and recent utility bill.

Following guidance from NHS England the BMA and RCGP our Doctors will not be carrying out the following during the outbreak of COVID-19 INSURANCE REPORTS AND CLAIM FORMS, NON-URGENT PAPER WORK AND DVLA MEDICAL EXAMINATIONS. This service will resume once the outbreak of COVID-19 is over.
